Better handling of player controls while paused. When pause ends, the ccmd "resetctlaccum" is called, which clears all accumulated player control relative offsets and impulse counts, ignoring any input the player may have generated during the pause.
Removed the old buttoncode_t completely (which contained, e.g., BT_CHANGE and BTS_SPECIAL).
The jump control is now working as an impulse.
