libcommon|Refactor: Use DOOM's save header format for Hexen

All supported games now use same format, version and read/write
logic for the serialized save header.

This means the v10 Hexen headers now support variable length save
names and include gameId, gameMode and mapTime values.

Older saves are augmented with suitable values (where missing).
